AS OF 4/2024 ALL NEW SPORTS PHYSICALS MUST BE ON

THE NEW OSAA PHYSICAL EXAMINATION FORM, REVISED 4/23.

PHYSICALS OBTAINED AFTER 4/24 AND SUBMITTED ON THE OLD FORM

WILL NOT BE VALID. 

 

OSAA PHYSICAL EXAMINATION FORM, REVISED 4/23

Physical Examinations

Students registering for Athletics must have a current OSAA physical examination form on file in the Athletics Office before the student is authorized to participate in practice/tryouts (dates listed below). To ensure clearance for participation, physicals should be completed and uploaded to the registration software no later than 3 school days prior to the first day of OSAA official practice/tryouts.

Fall: August 19, 2024
Winter: November 18, 2024
Spring: March 3, 2025

Physicals are valid for 2 years from the date of the exam. All physicals obtained after 4/30/2024 will need to be on the new OSAA form.
